本籍地（番地まで分かれば） Permanent domicile (street address if known) 戸籍取得に必要。出身地と異なる場合あり Required for family register. May differ from birthplace
生年月日（または大まかな年代） Date of birth (or approximate era) 例: 昭和5年（1930年）頃 e.g., Around 1930 (Showa 5)
現在のご遺灰の保管場所 Current location of the ashes 例: ラハイナの自宅 e.g., At home in Lahaina

あると助かる情報 / Helpful Helpful Information

宗教・宗派 Religion / Denomination 例: 曹洞宗、真言宗、キリスト教、神道など e.g., Soto Zen, Shingon, Christian, Shinto, etc.
お墓の場所の記憶 Memory of the grave location 寺の周辺環境、駐車場の大きさ、入口からの距離など Temple surroundings, parking lot size, distance from entrance, etc.
墓石の形や大きさ Gravestone shape and size 和型、洋型、大きさの印象など Japanese style, Western style, size impression, etc.
日本にいる親戚の情報 Information about relatives in Japan 連絡先があればベスト、名前だけでも可 Contact info is best, but just names help too
